The MDH Family Clinic Bushnell-Prairie City Jr. High and High School Sports Physical is set for Wednesday, July 16, 2025.
Attention all B-PC students who plan to participate in sports during the 2025-2026 school year: You must pick up your physical form at the Jr. High or HS office, or print a copy from the B-PC school website prior to the sports physical date. You must fill out your section of the form. A parent signature is required. Incomplete forms WILL NOT be accepted.
Students will not be allowed a sports physical without the completed forms.
Please make time available to have your sports physical on July 16th at MDH Family Clinic located at 1150 N. Cole Street at the times listed: Junior High Students (Entering 5th,7th and 8th ONLY): Noon - 2pm High School (Entering 10th-12th ONLY): 2:00-4:00pm.
**Students entering 6th and 9th grade require an office appointment with their primary care provider for a complete school physical.** Please have $10.00 in Cub or Check. No medical cards will be accepted. Please do not wear restrictive/tight clothing. Please call the B-PC HS School Office 309-772-2113.
